Carrington House Surgery News

NHS England have made an announcement in the press on 14th May regarding face to face appointments and open reception for patients.  Please bear with us whilst we put our systems in place.  For clarity, our Clinicians will continue to decide following a telephone consultation if you will need to be seen face to face.

National data opt-out

The deadline to opt out of NHS digital sharing your identifiable patient data is the 1st September 2021.
 
Any data that NHS Digital collects will only be used for health and care purposes. It is never shared with marketing or insurance companies.
 
If you wish to opt out of this, you will be required to fill out a Type 1 Opt-out form. This can be found online by clicking on the below link: https://digital.nhs.uk/services/national-data-opt-out Please return this form to the surgery.
If you are completing this for a child under the age of 13, you will be required to fill out the below form:
This needs to be emailed directly to the email listed on the form by the child's parents/carers

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. All new patients are required to provide proof of identification such as a passport, NHS card or birth certificate and proof of address such as a recent utility bill or bank statement.  

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. If needed, our receptionists will help you fill in the registration forms.

Change of Circumstance

To avoid your unintended removal from the practice list please notify us as soon as possible of any changes to your name, address and/or telephone number, both home and mobile.

  • HC2 Form (full help)
  • HC3 Form (partial help)

If you do not have a HC2 or HC3, we hold application forms at reception.
